Client Outcome Measures and Data Reporting
Overview
As part of implementing ACT and FACT, practitioners will complete a Client Outcome Assessment Battery (COAB) with clients when they enroll with the team, every 6 months while they continue to work with the team, and when they discharge from the team. Completing outcome measures helps ensure that ACT and FACT are improving clients’ lives in real and meaningful ways. Measures can also benefit your team’s everyday clinical practice by serving as tools for developing personalized treatment plans and helping the team identify areas where treatment is benefitting the client, as well as areas that may need more attention. The following trainings are designed to support practitioners in collecting and reporting client outcome data using the COAB.

Client Outcome Assessment Battery (COAB) Data Shell Orientation
This training module provides an overview of the Client Outcomes and Assessment Battery (COAB) Data Shell. The session explains the purpose and use of the data shell, describes its structure and organization, and provides guidance on entering client data, including a step-by-step walkthrough of transferring information from the paper COAB into the COAB Data Shell. Participants will also learn best practices for maintaining the data shell and explore available resources to support accurate ACT and FACT data collection and reporting.
Completing ACT and FACT Member Outcome Measures: Foundational Skills for Providers
This training is designed for California providers delivering Assertive Community Treatment (ACT) and Forensic Assertive Community Treatment (FACT). It provides a detailed overview of ACT and FACT client outcome measures required by the state of California, including the purpose and format of each measure and strategies for completing measures efficiently and effectively. The training also discusses how outcome measures can benefit treatment planning and client care, as well as best practices for communicating with clients about outcome measures.
Paper Client Outcome Assessment Battery (COAB) Orientation
This training module provides an overview of the paper Client Outcomes and Assessment Battery (COAB). This session explains where to locate paper COAB forms on the ACT and FACT Center of Excellence website, describes the structure and organization of the COAB, and reviews key instructions for accurate completion. It also highlights best practices for maintaining the COAB, as well as resources available to support ACT and FACT teams in collecting client outcome measure data.
