ACT and FACT Transitions in Care
Overview
This training will focus on clients who transition from Assertive Community Treatment (ACT) to a lower level of care. Participants will examine the layered issues that underlie these transitions from an intensive program to a less intensive program, from a graduation that resulted in a client achieving their goal(s) to a disengagement, where a client may experience an ambivalent separation from the ACT program. Participants will review existing decision-making tools and readiness scales that not only inform transitions from ACT but also address provider concerns about transitions in ACT. The training will also review a best-practice approach in the form of a clinical scenario that explores the application of a transition. The aim of this training is to prepare clinicians to utilize high-quality strategies for transitioning clients to a lower level of care when clinically appropriate.
